Proactive Puppy Classes Application Form

FOR PUPPIES AGED 8 - 20 WEEKS OLD

Classes are 60 minutes long and run once a week over 4 classes

DATES - Wednesday 8th October, 15th October, 29th October and 5th November for Graduation (Guy Fawkes!)

TIME - 6:30pm - 7:30pm

LOCATION: Lollipups Dog Daycare - Napier, 97 Niven Street, Onekawa

Each class is limited to 8 puppies and they must have had their first vaccination at least one week prior to starting classes.
